There are some recent posts over on the ASGS facebook page you should all have a look at that demonstrate proper installation of regulators and accidents that happen with O2 cylinders - it's a reality check for sure. Check them out at

Of particular interest is the potential for O2 cylinder tank explosions resulting from contaminants in the regular that can be ignited when the high pressure gas is introduced into the regulator too quickly - termed heat of compression.
I also found this excellent safety data sheet from Harris. Print it out and keep it next to your tanks - oh and read it a couple of times a month to keep the information fresh in your mind at all times.
